What mostly fueled his dramatic wealth creation, besides what has already been mentioned, is his use of OPM (well not exactly OPM).

His use of all the insurance company premiums that roll in every month/quarter/year, under the BH umbrella, to fund stock purchases. Not to mention DC lobbying.
